Overview Job Summary: Within a team environment, provides person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance.

Responsibilities

Support individuals we serve by maintaining regular contact and doing Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).

Participate in Interdisciplinary Team (IDT) staffings and attend meetings in the community as needed, including Individual Education Plan (IEP) and day program meetings.

Provide information and referral to generic services and resources in the community, including schools, recreational options and local support groups.

Supportive Employment/Habilitation funded day programs and local agencies (C.C.S., Dept. Rehabilitation, Social Security, Medi-Cal, IHSS).

Engage with specialist physicians/specialists (speech therapist, O.T./P.T.).

Coordinate and secure Tri-County Regional Center funded services, including behavior services, training for independent living, supportive living, out-of-home placement, in-home and out-of-home respite, infant and adult day programs, durable medical equipment and specialized non-Medi-Cal funded services.

Develop and maintain a network of vendors and community agencies; provide training about developmental disabilities, services and resources.

Design, document, implement and monitor services and supports in a timely manner; facilitate planning team progress, identifying outcomes and needed services.

Involve individuals we serve and family in all aspects of person-centered planning and support activities.

Qualifications and Requirements

Knowledge of developmental disabilities; awareness of entitlement and public benefit programs.

Problem-solving ability; strong verbal and written presentation skills.

Organizational and time management skills; ability to work independently and within a team; responsive to supervision.

Able to understand and adhere to applicable laws, regulations, policies and procedures.

Ability to read/speak/write Spanish and English (Required).

Minimum Qualifications:

Master’s or Bachelor’s degree in a human services field or related area, with 2 years of applicable experience in mental health or social work; 2 years experience in case management within a related field; 1 year experience with individuals with developmental disabilities preferred.

Valid California driver’s license and reliable, insured vehicle or equivalent means of transportation; able to travel to any location at any time.

Able to read/speak/write Spanish and English (Required).

Physical Requirements Adequate manual dexterity and coordination for operation of a computer and other standard office equipment; ability to sit at a workstation for long periods; ability to travel to another work-site location daily; ability to stand, sit, walk for moderate distances on uneven terrain. Regularly required to use hands, fingers, and arms; may lift up to 10 lbs regularly and up to 20 lbs infrequently. Vision requirements include close, distance, color, depth perception and focus adjustment.

Salary Bachelor’s degree: Starting Salary: $25.97 - $28.62. Full Salary Range: $25.97 - $31.60

Master’s degree: Starting Salary: $30.07 - $33.14. Full Salary Range: $30.07 - $37.60

+$110 biweekly bilingual stipend
