Overview
The primary purpose of this position is to manage the agency's LINKS plan in accordance with the Chafee Foster Care Independence Act. The LINKS program involves working with adolescents, teens, and young adults ages 13 to 21 to improve outcomes regarding pre-martial pregnancies, school dropout, criminal records, incarceration, and involvement in other high-risk behaviors, once they leave foster care. An essential function of this position requires operating a county vehicle in the performance of work. This employee will coordinate, facilitate and monitor program activities to ensure that the participants develop and maintain self-sufficiency skills. Responsibilities
Conduct annual written assessments of each participant age 13 and older in agency custody regarding their strengths and needs related to self-sufficiency. Develop a transitional living plan with each participant that reflects the goals of the self-assessment and incorporate it into the agency service plan (case plan). Perform other required activities according to policy and collect data on current and former LINKS participants, submitting reports to the State LINKS Coordinator. Provide training to existing child welfare social workers, new hires, and foster parents. Provide LINKS Transition and Services to Youth in Foster Care, ages 18 to 21, including housing assistance coordination, skills training (money management, home repairs, conflict management, legal rights and responsibilities in housing), mediation between the young adult and landlord, and determining eligibility for Transitional Housing Funds. Deliver LINKS services to youth/young adults ages 13 to 21, including life skills assessments, development of individual contracts, resource exploration, service documentation, and coordination of inter-county events, while maintaining communication with the LINKS State Coordinator and program liaisons in other counties. Collect and analyze data on specified outcomes, prepare surveys, annual reports, and applications for LINKS funding, and ensure reporting requirements to the State LINKS Coordinator are met. Engage in Outreach Services to locate and connect at-risk youth and young adults with LINKS services, including community networking and information dissemination about available resources and eligibility. Provide training to current Child Welfare staff, new hires, and foster parents; participate in local, state, and national trainings and conferences relevant to LINKS services. Perform other duties as necessary. Qualifications and Skills
This position requires thorough knowledge of basic social work principles, techniques, and practices and their application to complex casework, group work, and community problems; knowledge of governmental and private organizations and resources in the community; some knowledge of behavioral and socioeconomic problems and their treatment; knowledge of NC statutes related to child abuse/neglect reporting law; and familiarity with social services, protective services, foster care services, and child care services. The employee must be able to establish and maintain effective working relationships with supervisors, caseload families, care providers, and community organizations; communicate ideas clearly; plan and execute work; and handle hostile clients.
