A Brief Overview Ambulatory Social Workers are professionals trained to provide social work/case management services to patients, their families, and their support persons/systems in both ambulatory and inpatient settings. This includes the professional activity of assisting individuals, families, groups, organizations or communities to enhance or restore the patient's capacity for social functioning and to create societal conditions favorable to their goals.
Experience Qualifications
Experience in customer service. Preferred
Clinical and/or medical social work experience. Preferred
Experience in group work, case work, and community methodologies. Preferred
Licenses and Certifications
Licensed Bachelor Social Worker – KSBSRB temporary licensure may be accepted but full licensure must be achieved within 6 months of hire. Required
Licensed Masters Social Worker – KSBSRB temporary licensure may be accepted but full licensure must be achieved within 6 months of hire. Required
What you will do
Participate in patient admission process (Inpatient, Outpatient, PHP, IOP) a) assist as needed with signing of voluntary papers, b) serve as hospital representative and patient advocate.
Coordinate and facilitate services for patients and their families throughout the entire Stormont Vail Health system.
Perform social work assessments, demonstrating awareness of the bio-psychosocial, cultural, developmental and spiritual ramifications of illness and hospitalization.
Develop appropriate case management and/or treatment plans based upon assessment, client/family/caregiver specific concerns and recommendations of multidisciplinary care team.
Address patient/family/caregiver understanding of and adjustment to the illness, to maximize benefits of intervention and enhance patient/family/caregiver functioning. Establish supportive and therapeutic working relationship with patient/family/caregivers. Communicate in a clear and concise manner with patients/families/caregivers regarding patient status and progress. Adjust communication style responsive to receiver.
In the process of restoring the patient’s capacity for social functioning: initiate discharge planning upon admission; ensure plans are coordinated with internal and external multidisciplinary teams and implemented; initiate and develop long‑term planning designed to reduce recidivism, referrals as indicated.
Develop and facilitate groups for patients and/or significant others as needed.
Maintain professional levels of productivity and coordinate and prioritize patient care related activities in an efficient and effective manner.
Provide problem solving and leadership in the development of new services which respond to patient care demands.
Maintain confidentiality and comply with professional ethics according to professional, departmental and Medical Center standards.
Actively participate and initiate when appropriate in patient care conferences.
Provide patient education regarding diagnosis and treatment.
Provide crisis and/or brief therapy for individual, family and group.
Assess patient and family functioning at time of discharge or referral to another provider.
