Overview The Collaborative Care Case Manager (CCCM) will provide services for up to 15 individuals in Bluebonnet Trails Community Services’ Outpatient Biopsychosocial Services for Persons with IDD Program (OBI). The program assesses, offers treatment services, and makes referrals to needed services and supports in a biopsychosocial framework, addressing the whole person. The CCCM coordinates case management with mental health providers and supports the individual and their family/support system in understanding co-occurring mental health and/or substance use diagnoses, increasing involvement in recovery, and providing support during a crisis.

Responsibilities

Provide case management and collaborate with mental health providers to support individuals with IDD and their families/support networks.

Identify and address barriers to accessing services and provide resources to ensure appropriate service delivery.

Make referrals for needed mental and physical health services and follow up on referral results to support ongoing recovery.

Facilitate development of a person-centered, strength-based treatment plan with the individual and their support system to guide goals and skill development.

Ensure timely follow-up when individuals are hospitalized in psychiatric facilities or incarcerated, to begin discharge planning or diversion where possible.

Serve individuals of all ages who reside in Williamson or Burnet Counties. Services may be provided via tele-video, face-to-face, or telephone as appropriate.

Work with families and demonstrate sensitivity to cultural differences; build relationships within the agency and with external partners to support duties.

Participate in data gathering and timely reporting as directed by the supervisor.

Williamson and Burnet Counties

Hourly Salary Range:

$21.46 – $30.21

Up to $2.00/hour for prior experience in a similar role with a community center in Texas

Up to $4.00/hour – Tenure with BTCS

$0.75/hour – Bilingual

$1.00/hour – Masters Degree in Human Services

$1.00/hour – Advanced Licensure

Eligibility for $200/month Service Delivery Incentive Award

Qualifications Minimum Qualifications:

Bachelor’s degree from an accredited college or university with a major in a social, behavioral, or human service field (including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education or criminal justice). Must have at least one year of experience providing services to individuals with intellectual disabilities. Must have a valid Texas driver’s license and maintain approved driver status throughout employment.

Preferred Qualifications:

Two-three years of experience providing services to individuals with intellectual disabilities; experience providing mental health services; experience providing crisis services; bilingual (Spanish/English or ASL/English); experience providing services to persons with substance use disorders; experience providing case management or service coordination.
