Physical Therapist – Inpatient Per Diem Baystate Health is seeking a Per Diem Physical Therapist to join the Inpatient Rehabilitation team in Greenfield, MA. New grad applicants accepted. Baystate Franklin Medical Center (BFMC) is an 89-bed community hospital in Greenfield, MA offering services including emergency, surgery, cancer, heart and vascular, obstetrics, and more. Treats patients using standard physical therapy procedures. Determines patients capabilities, needs, interests, and goals and prepares individually tailored programs consistent with the physician's diagnosis. Expected to function independently at this level. May perform other similar and related duties as assigned. Responsibilities Evaluate assigned patients, develop plan of treatment, provide assessment, and assist in discharge planning. Complete evaluations within 24 hours; plan goals and treatment; assist patient/family with equipment and discharge planning with rehab team. Apply treatment plan; modify as needed; train patients and family on carryover procedures and equipment; maintain a safe environment; demonstrate empathy. Complete necessary documentation for initial consult, progress notes, and discharge reports; document within 24 hours with history, results, prognosis, and recommendations. Provide information and counseling to patients, families, and other health care personnel involved in management; coordinate with MDs, nursing, and ancillary services; document appropriately. Manage department operations including scheduling, billing, equipment usage, and caseload coverage; submit charges within 24 hours; provide staff coverage as needed. Assist with on-the-job orientation and training for student interns; contribute to intern evaluations and rating. Demonstrate knowledge and application of clinical skills; set annual professional development objectives. Meet productivity expectations and participate in in-service training and seminars. Attend and present at in-service training as directed.
