Registered Nurse (RN) – Senior Health and Wellness
to support public health initiatives focused on senior health, wellness outreach, and home-based care coordination. Under the supervision of the Director of Nursing and the Public Health Education and Nursing (PHEN) Director, this role centers on improving quality of life for older adults, supporting families with medically complex children, and enhancing access to preventative services across the community. This is a
community-based nursing role
that offers variety, autonomy, and the opportunity to make a real difference in the lives of families across Knox County. Key Responsibilities
Senior Health & Wellness Outreach (Primary Role) Conduct health and safety assessments for seniors to support independence and well-being. Provide informal counseling, social support, and care coordination to older adults. Connect seniors with community resources and services to address physical, emotional, and social health needs. Collaborate with partner organizations to expand outreach and strengthen service networks. Complex Medical Help (CMH) Program (Secondary Role) Conduct home visits for children with complex medical needs. Assess clients comprehensively and re-evaluate as necessary. Develop, implement, and revise individualized care plans. Educate clients and caregivers on disease prevention and care techniques. Facilitate enrollment in CMH, Women, Infants, and Children (WIC), and other support programs. Obtain and maintain physician orders and appropriate medical documentation. Coordinate care with multidisciplinary teams (physicians, therapists, aides, etc.). Ensure services are billable under CMH public health nurse consultative services. Refer eligible families to the Help Me Grow program. Additional Public Health Nursing Duties Provide education and resources for families of children with elevated lead levels; ensure follow-up testing with medical providers. Stay informed on local services; make and document referrals to appropriate community programs. Coordinate homebound vaccine visits for Knox County residents and assist with occasional out-of-county requests. Administer and document immunizations at agency clinics or outreach events. Update and reconcile data in the statewide immunization registry as needed. Serve as backup nurse for:
Newborn home visits (including documentation and follow-ups in Electronic Medical Records (EMR)) Infant Safe Sleep Program Communicable disease investigations
Qualifications
Bachelor’s degree in Nursing (BSN)
– required Licensed RN in Ohio
– required Minimum 1 year of RN experience
– required Active BLS (Basic Life Support) certification
– required Proficient in
EMR
systems Excellent communication, customer service, and organizational skills Proficient in
Microsoft Office Suite
(Word, Excel, PowerPoint) Valid
Ohio Driver’s License
and personal vehicle insurance Strong multitasking skills and ability to manage priorities Preferred Experience Experience in community or public health nursing Knowledge of Ohio CMH and WIC programs Familiarity with infant and child health programs Why Join Us?
