Overview CP Rochester and Happiness House are private nonprofit health, educational, residential and human service organizations that have been serving communities in the Finger Lakes Region since 1969 and the Greater Rochester Area since 1946. Today with sites in Rochester, Canandaigua, Geneva, Gorham, Midlakes and Waterloo, our agencies offer an array of educational, therapeutic, recreational, residential and family support programs and services for children and adults with and without disabilities. On average, our agencies serve 3,650 families each year and employ approximately 600 people.

Description Job Title:

Physical Therapist – Children’s Programs

Location:

CP Rochester – Winton Road Facility

Annual Salary:

$58,900 - $72,000 commensurate with experience

This position is eligible for a $3,000 signing bonus!

This bonus is earned and payable as follows: 25% ($750) in the pay period following the 3-month anniversary of your employment; 25% ($750) in the pay period following the 6-month anniversary of your employment; and 50% ($1,500) in the pay period following the one-year anniversary of your employment.

Hours of Employment:

This is a full-time 37.5 hours per week position. Days and hours of work are Monday through Friday during business hours. Evening work may be required as job duties demand. Vacation time is to be taken during the school breaks and PTO is allowed to be used as needed.

Requirements

Bachelor’s degree in Applied Science. Completion of accredited Physical Therapy program.

License or eligible for license as a Physical Therapist in New York State.

Current New York State registration as a Physical Therapist.

Valid New York State driver’s license.

Summary/Objective CP Rochester and Happiness House exist to support individuals with physical and developmental disabilities in choosing and accomplishing successive individualized life goals. We do this by providing a wide range of services to people of all ages in the greater Rochester area. Our purpose is to ensure that those we serve safely determine their own pathways in life and live as full members of their communities.

Essential Functions

Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources.

Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.

Upholds NYS Education Law/Practice Act as relating to the practice of physical therapy.

Performs appropriate evaluations to individuals to determine the need for physical therapy services.

Ensures valid prescriptions are written for all individuals on caseload.

Carries out treatment plans developed after evaluation in the Preschool or School Programs, as necessary.

Available to consult with other professionals such as teachers, habilitation staff, as well as other members of the interdisciplinary team as appropriate, to facilitate delivery of coordinated quality services to individuals.

Works with parents/legal guardians as integral members of the interdisciplinary team in the school setting, as needed.

Communicates with physicians and other medical personnel in order to facilitate evaluation and treatment of individuals.

Completes all paperwork in accordance with agency, program and department policies/guidelines.

Ensures knowledge of paperwork, Electronic Health Record and applicable Medicaid regulations in order to provide appropriate documentation.

Attends all team, discipline and staff meetings as designated.

Maintains treatment area in an orderly manner to ensure safety.

Maintains all department equipment as appropriate.

Advises Supervisor of need for replacement of old or ordering of new equipment.

Meets with interdisciplinary team and Program Supervisor as needed to review status and need of program modifications for individuals on caseload.

Participates in conferences, workshops and in-service training as needed or recommended to enhance job skills and personal growth and development.

Adheres to agency’s policies and procedures as well as department guidelines.

Complete and maintain CPR and First Aid Certification

Complete a Level 3 Life Guard Certification Course

Supervisory Responsibility None. Potential to supervise Physical Therapy Students from local colleges and universities.

Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to sit, stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move objects up to 10 pounds and occasionally lift and/or move objects up to 50 pounds. Ability to pull and push wheelchairs up to 250 – 300 pounds – maximum weight required.

Travel is primarily local during the business day but employee will be expected to work at any of our partner agencies. Some out-of-the-area and overnight travel may be expected
