Job Description: Position Summary. Responsible for providing psychotherapy and counseling therapies including group, individual and family therapy for patients suffering from psychiatric and substance abuse issues. Ensures accurate and timely patient documentation.
Qualifications
Education:
Master’s degree in social work or counseling or relevant state licensure required.
Experience:
Prior experience with psychiatric and chemical dependency patients.
License:
Current unencumbered clinical license per state of practice guidelines.
Additional Requirements:
CPR certification and Crisis Prevention Training (CPI) within 30 days.
Responsibilities
Conduct individual and group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problems.
Display active involvement in the treatment planning process.
Display active involvement in discharge, transition, and after-care planning.
As therapeutically necessary, provide family session counseling to ensure appropriate communication and involvement of family members and support groups.
Support with vital and UDS capture as needed.
Actively communicate with clients, families, and outside referral sources, functioning within the scope of HIPAA and 42 CFR Part 2. Document all correspondence with patients and stakeholders.
Ongoing and consistent collaboration with the interdisciplinary team including physicians, utilization review and nursing staff. Demonstrate active communication with team members.
Effectively communicate to UR on treatment and LOS issues, supporting in UR functions/authorizations/denials as needed and requested.
Complete appropriate paperwork for clients in a timely, accurate, and complete manner.
Complete required assessment paperwork including patient assessment scales, Columbia, psychosocial/LOC assessments (not applicable at all facilities), psychosocial/LOC updates, and assigned assessment paperwork.
Demonstrate appropriate crisis intervention and de-escalation skills. Act as a team member during crises to ensure patient safety.
Enact the code system per company policy and training.
Complete accurate assessments of patients utilizing clinical skills. Assist care coordination department as requested.
Complete all documentation thoroughly, timely, and in accordance with hospital/facility standards.
Display a thorough knowledge of confidentiality. Complete releases appropriately. Demonstrate knowledge of HIPAA and 42 CFR Part 2 regulations and abide by those standards.
Demonstrate a working knowledge of all clinical regulations, including TJC/CARF (as applicable), Medicaid, and state and federal laws.
Other duties as assigned by leadership.
Physical, Mental, and Special Demands
Ability to sit and stand (one-third of the shift) and walk (two-thirds of the shift).
Ability to reach above, or below waist height, kneel, bend, stoop, turn, and twist (occasionally to frequently).
Visual ability to read charts, treatment plans, computer screen and typewritten materials.
Ability to frequently lift and carry 10 lbs.; occasionally 20 to 50 lbs.
Ability to work in an environment with chemicals used for cleaning, with potential medical and electrical hazards, and where dust, mist and steam are generated in housekeeping tasks.
Ability to work at a rapid pace and perform a variety of repetitive duties.
Ability to respond to exposure to blood and body fluids.
