The Physical Therapist is responsible for providing medically prescribed, goal-oriented therapy in accordance with the
Physician Plan of Care , company policies, and professional standards of practice. This role also involves coordinating care delivery and collaborating with the interdisciplinary team to ensure the highest standards of patient outcomes. Develop and implement individualized patient physical therapy plans of care. Provide in-home visits, evaluating and reassessing patient needs. Communicate with physicians and care team members as needed. Deliver education and instruction to patients, families, and team members. Participate in interdisciplinary team meetings to review and coordinate care. Ensure care plans are regularly reviewed, updated, and documented accurately. Adhere to all agency policies, Federal/State regulations, and ACHC accreditation standards. Complete electronic documentation timely and accurately using the EMR system. Required Experience, Education, Skills & Abilities
Current Physical Therapist (PT) license in the state of Florida and in good standing. Specialized training/experience in
pelvic floor therapy . Knowledge of physical therapy principles, concepts, and methodologies. Excellent written, verbal, and interpersonal communication skills. Strong problem-solving and decision-making abilities. Ability to work effectively with an interdisciplinary healthcare team. Current CPR certification Why Work With Us?
