This position requires working three, 10-hour days and one 7.5-hour day (37.5 hours per workweek). The schedule will consist of three consecutive days from 8:30am to 7:00pm and one day for 7.5 hours between 8:30am-7:00pm. Work schedules could include one or both weekend days (Saturday/Sunday) and are subject to change to meet client care requirements.

Pay Range (Dependent on experience):
$31.54 - $38.34/hour

Description The Case Manager will support the Enhanced Care Services (ECS) program within the Adult Mental Health Department. This position assists clients who may be experiencing significant mental and behavioral health symptoms and who may require a high level of attention and care.
The Case Manager provides case management services to assigned clients. This position assesses eligibility, assists clients in accessing services, and collaborates with Agency professionals in case planning and service coordination.
Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are trauma-informed and recovery-oriented. MCCFL serves Hood River, Wasco, and Sherman Counties and uses a consumer-involved, consumer-engaged, and consumer-driven model. MCCFL offers a whole-person care approach to adults and children to address both physical and mental health needs, including programs such as mobile crisis, intensive case management, counseling, supported employment, jail diversion, hospital discharges, and more.

For Case Manager Levels II:
Two (02) years of responsible experience in human services or related field is preferred.
Experience working with the severe and persistent mentally ill population is preferred.
Bilingual fluency in Spanish/English is preferred.
Must possess a valid Oregon or Washington Driver’s license and be insurable for client service purposes and travel between offices, meetings, and trainings.
Must pass all criminal history check requirements as required by ORS 181.536-181.537 and OAR 407-007-0200 to 407-007-0370.
Additional Specifications for Level II:
Certified as a Qualified Mental Health Professional (QMHP) by MHACBO or register as QMHP-R within 14 days of hire.
Master’s degree in a MHACBO-qualifying field.
What you'll do
Provide Case Management services to all assigned clients. Services are provided in a variety of community settings, with consideration to client preference.
Assess client eligibility, assist enrolled clients in acquiring services and resources, and coordinate with MCCFL and community partners to develop services and resources.
Participate in medication appointments, medication delivery, and accurate and timely documentation in client records.
Participate in joint case planning and coordination with MCCFL multi-disciplinary teams and agency psychiatrists.
Meet minimum client service productivity requirements as set by the Agency.
Utilize agency clinical software to prepare current client records and reports; maintain timely and coherent documentation; ensure documentation is up-to-date and justifies the services provided.
Adhere to all clinical and MCCFL program policies and procedures; stay updated on applicable rules and OARs.
Participate in team and agency meetings; attend local, regional, and state meetings as requested by Supervisor/Manager.
Work independently in the community and promote a positive image of the agency.
Maintain understanding of local behavioral health resources and MCCFL’s role in the system.
May provide after-hours emergency services on a 24-hour rotating basis as needed; duties may be required as an essential responsibility based on budget and staffing.
May provide administrative support to the team (e.g., filing, greeting clients, screening calls).
Use agency vehicles to travel within the community, transport clients, and drive between offices for meetings/trainings.
Maintain regular attendance and professional communication; uphold client confidentiality.
Additional Responsibilities
Conduct behavioral health screenings and/or mental health assessments for clients.
Determine and document client diagnoses.
Assess the client’s family, social, and work relationships and provide supportive services where appropriate.
Create and oversee an individual plan of care.
Position-Specific Responsibilities (ECS Program)

Participate in daily psych therapeutic groups.
Work closely with community partners, including Hood River Care Center staff.
Develop specific behavior plans using positive reinforcement methods.
Travel between agency offices and locations throughout the community.
