Qualifications

Licensed Speech-Language Pathologist. Open to applicants of all experience levels.

Key Responsibilities

Screen and evaluate patients with communication, cognitive, or swallowing disorders and develop appropriate plans of care following regulatory and clinical practice standards. Provide rehabilitative, skilled, and medically necessary treatment interventions for communication, cognitive, and/or swallowing disorders per regulatory and clinical practice requirements. Evaluate treatment outcomes and modify services as needed; make referrals based on outcomes. Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders. Provide comprehensive discharge summaries of services rendered per regulatory and clinical requirements. Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
