Fred Finch Youth & Family Services partners with individuals and communities to provide culturally responsive services that cultivate mental and physical resilience and wellness.

The Enhanced Care Management (ECM) Program, operating under the umbrella of Fred Finch CARES, is a comprehensive care coordination benefit launched in July 2023 under CalAIM (California Advancing and Innovating Medi‑Cal). ECM adopts a Whole Person Care Approach to healthcare, providing physical, behavioral, emotional, and social services to enhance care coordination and health outcomes. It is a care‑coordination service offered to children, youth, and their families insured through Managed Medi‑Cal. With a cross‑regional services delivery model in both Northern and Southern California, ECM aligns and coordinates services across multiple systems of health and behavioral healthcare while addressing social determinants.

What You Will Do The CARE Manager oversees specific cases, coordinates health care benefits, provides education and facilitates client access to care in a timely and cost‑effective manner. The CARE Manager collaborates and communicates with clients, caregivers/family support persons, and other providers to promote wellness, recovery, independence, resilience, and empowerment, while ensuring access to appropriate services and maximizing client benefit.

The CARE Manager also serves as an advocate for clients, an active client of the interdisciplinary team, a liaison with other programs and external health and social service providers in the community.

Qualifications

A minimum of three (3) years of relevant work experience.

High school graduate or GED.

Must be bilingual in English/Spanish.

Long‑time resident of the community with knowledge of local resources.

Ability to complete the training program and ongoing educational requirements as assigned.

Ability to work flexible hours, including occasional night/weekend work.

Interpersonal savvy, demonstrated by the ability to interact with and influence people to establish trust and build strong relationships.

Ability to perform home visits and outreach.

Experience with usage of EMR/EHRs/Care Management software.

Strong organizational skills and ability to manage and maintain a personal schedule.

Ability to establish priorities and meet deadlines.

Ability to work independently within a virtual operating environment and as part of a team.

Excellent oral and written communication skills.

Ability to exercise judgment in the application of professional services.

Active listening skills and genuine compassion for others.

Enjoy collaboration within a team environment and working with people of different skills and experience.

Knowledge of community resources in the local market.

Able to maintain clear professional boundaries with patients and coworkers.

Commitment to represent the company with professionalism.

Demonstrates cultural competency and ability to work with diverse groups of community clients.

Comfortable using computer for documentation, communication, and organizing work.

Demonstrated cultural responsiveness in working with diverse families.

Must be able to respond quickly to emergencies, and to learn and implement Pro‑ACT (or other agency approved crisis de‑escalation training).

Clean driving record: current CA Driver’s License or ability to obtain within 10 days after.

Must have access to dependable transportation; personal car insurance meeting minimum liability coverage required if position requires driving for work or transporting.

Basic computer and keyboarding competence with the ability to use Microsoft Office Suite and/or competency with electronic clinical documentation.

Preferred

Multilingual capabilities. Must be bilingual in English/Spanish.

Prior experience as a Community Health Worker, Peer Support Specialist, Medical Assistant or similar role.

Fluency in threshold languages.
