4West is a high acuity 21-bed progressive care unit that provides specialized nursing care for adult patients that require various oxygen therapies including BIPAP therapy, administration of titratable and non-titratable medications as well as administration of blood products. Cardiac patient diagnoses include post cardiac catheterization, myocardial infarctions, and congestive heart failure exacerbations. Pulmonary patients may have chronic respiratory diseases including cystic fibrosis, tracheotomy, interstitial lung disease, pulmonary emphysema, pulmonary fibrosis and related conditions. Conducts systematic, continuous and comprehensive assessments of patient health status from admission to discharge, and collaborates with the healthcare team, patient and family to ensure safety and address age, gender, spiritual, cultural, physical and psychosocial needs. Independently administers medications per licensure authority, assesses effects, documents administration timely, and adheres to patient safety guidelines. Collaborates with patient and family to plan care using current scientific knowledge and standards; creates individualized, age-specific plans and ensures continuity and appropriate patient teaching. Identifies and prioritizes patient problems based on assessment data, safety considerations, and patient/family goals and preferences; applies nursing interventions accordingly. Implements nursing interventions to meet patient and family needs while demonstrating clinical judgment and safety. Assesses and evaluates patient/family teaching for effectiveness, adapting to age and cultural needs. Evaluates care by reviewing patient/family responses and revising the plan of care with the care team to promote outcomes and safety. Advocates for patients and families, prepares for discharge, and seeks feedback to improve satisfaction. Works collaboratively with unlicensed assistive personnel under appropriate supervision to ensure safe patient care delivery. Shift Requirements
Shift Length: 12 hours Shifts/Week: 3 Overtime Required: Infrequently Education
Associate’s Degree in Nursing from an accredited nursing program is required. All new-hire associates must start an approved BSN program within one year and complete it within three years (required). Bachelor’s Degree in Nursing (preferred). Experience
All new RN graduates will participate in the Nurse Residency program (required). Two years of hospital patient care experience (preferred). Licensure
Current KY RN licensure or compact license with privileges to work in Kentucky (required). Certification
Basic Life Support (BLS) from the American Heart Association (required). Pediatric Advanced Life Support (PALS) may be required for specific clinical areas. Advanced Cardiovascular Life Support (ACLS) may be required for specific clinical areas. Note: additional, specialized certifications may be required or preferred based on unit needs.
