Physical Therapist I - Outpatient Pediatric Therapy

Job Summary Assesses and treats pediatric patients referred to the Physical Therapy department. Documents patient evaluation, plan of care, progress and discharge status.
Job Duties

Knows and adheres to all laws and regulations pertaining to patient health, safety, medical information. Complies with regulatory agency standards and regulations.
Conducts evaluations consistent with a diagnosis and designs a goal‑oriented treatment plan based on interpretation of results and patient goals.
Assists patient toward goal achievement through effective treatment plan and interventions. Administers individual treatment procedures and develops home exercises and self‑treatment plans.
Provides education to patient and family, including diagnosis and prognosis of therapy. Recognizes adverse responses and adjusts treatment accordingly.
Makes timely and appropriate discharge plans and recommendations for anticipated patient needs.
Responsible for accurate and thorough completion of evaluation reports, progress notes/flow sheets, discharge summaries, and any additional documentation required within a specified time frame.
Participates in staff orientation, patient staffing and team meetings. Makes appropriate recommendations and referrals for additional services.
Maintains established departmental productivity standards.
Actively participates in departmental initiatives such as Performance Improvement Initiatives, CARF, JCAHO and State Board of Health readiness.
Provides direction to Physical Therapy Assistants by delegating appropriate patients and reviewing plans of care and providing clinical support as necessary. Provides day‑to‑day direction to rehab techs and volunteers.
Participates in student program as appropriate and based on level of experience and expertise.
Attends all mandatory in‑services and meetings and provides in‑service for staff after attending courses outside the organization.
Adheres to and maintains all necessary competencies required for job responsibilities.
Performs other duties consistent with purpose of job as directed.
Education Requires a minimum of a bachelor’s degree in Physical Therapy from an accredited academic program.
Licensure Must be licensed in the State of Indiana as a Physical Therapist.
Experience Prefer previous experience in a physical therapy clinical setting but will accept a new graduate from an accredited college.

Excellent communication skills, both verbal and written are required.
Must be able to adjust to varying treatment loads and schedules.
Requires flexible work schedule including weekends.
Considerable initiative, judgment and problem solving required.
