NURSE PRACTITIONER INPATIENT CARDIOVASCULAR HEART FAILURE

The Cardio Vascular Service Line Heart Failure Inpatient Nurse Practitioner utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Seeking an NP with cardiac clinical expertise in cardiac surgery, cath lab, CCU/ED. APP preferred to have private practice cardiology experience. Responsibilities

Perform and document accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender, and clinical problem. Order, perform, and interpret laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate. Manage general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and indications/contraindications of pharmacologic agents and other treatment modalities. Prescribe medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed. Seek opportunities to promote Hackensack Meridian Health (HMH) APN practice recognition locally, nationally, and internationally through conferences, publications, and presentations. Provide patient- and family-centered, compassionate, and effective care promoting health and treatment of disease. Practice as a healthcare provider according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges. Adhere to HMH nursing protocols, standards of care, and disease-specific quality indicators. Apply evidence-based and value-based medicine to systematically analyze and improve patient care practices and outcomes. Evaluate patient/family transition of care planning considering resource utilization, cost, quality, safety, and self-management promotion for chronic conditions. Demonstrate clinical expertise through advanced assessment and procedural skills pertinent to the practice specialty. Complete medication reconciliations as appropriate. Participate in annual APN peer review. Translate best practice into clinical practice by evaluating current protocols and standards of care. Provide culturally sensitive and educationally appropriate health management education to patients and families. Facilitate consistent, coordinated care and clear communication among all members of the healthcare team through participation in daily interdisciplinary rounds. Serve as an advocate for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate. Serve as a clinical resource for the multidisciplinary team by addressing clinical, process, and system improvement opportunities. Participate in organizational and departmental meetings, committees, and workgroups as necessary. Demonstrate the highest level of accountability for professional practice and adhere to ethical principles regarding patient confidentiality, informed consent, and adverse outcomes. Collaborate with medical staff to support nursing-led protocols. Mentor other APNs, precept students in APN programs, and participate in educational programs for nursing as a clinical expert. Identify patient/family educational needs and collaborate to implement appropriate teaching materials. Facilitate and participate in nursing educational programs as a clinical expert. Promote a collaborative work environment supporting the organizational mission, behavior expectations, values, and pillars. Promote innovation by integrating new clinical knowledge into practice. Participate in clinical research activities and disseminate findings through practice, education, presentations, and publications. Identify ethical care issues and coordinate interdisciplinary meetings to resolve them. Adhere to the legal framework and scope of practice defined by the NJ Division of Consumer Affairs, ensuring compliance with all applicable statutes and professional standards. Comply with HMH and department-specific scope of services, policies, and procedures as applicable to role. Other duties and/or projects as assigned. Adhere to HMH Organizational competencies and standards of behavior. Qualifications

Graduate of an NLN/AACN accredited nursing program. Master's or Doctorate degree from an accredited Nurse Practitioner program. Maintains clinical competencies through continuing education and clinical practice. Experience in a relevant patient care setting (or as regulations dictate). National Provider Identifier (NPI). Prescriptive authority through CDS and DEA registration according to New Jersey state law. Collaborative agreement with physicians on staff. Excellent written and verbal communication skills. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. NJ State Professional Registered Nurse License. Advanced Nurse Practitioner (w/ Master's Degree). BLS AHA Basic Health Care Life Support HCP Certification. Board certified by AANP or ANCC. Board certified by AANP or ANCC (preferred).

Advanced Cardiac Life Support Certification (preferred). Compensation
