Base pay range: $55.00/hr - $57.00/hr. This hybrid model supports outpatient rehabilitation services in an in-home setting for older adults, combining field visits with remote interventions and assessments part of the schedule. Responsibilities

Provide outpatient rehabilitation services in an in-home setting for older adults (patient residence or independent living facility room). Evaluate, develop, and deliver individualized physical therapy programs to facilitate rehabilitation. Identify needs for referrals to other disciplines and coordinate care with the rehabilitation team. Educate and train patients and caregivers to promote independence and reduce pathology. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as appropriate to licensure and standards. Adhere to department and patient treatment schedules, documentation, and meetings. Attend patient care and staffing conferences and contribute to related meetings. Order supplies and equipment; maintain and report equipment status and needed repairs. Promote a safe environment and eliminate hazards; participate in home and community mobility assessments and safety planning. Modify plans of care based on patient needs; collaborate with team to select appropriate therapies. Document and progress patient care, including tests, measures, goals, and plans, with emphasis on home-based and community mobility components. Provide health and wellness programming integrated into treatment plans. Qualifications

Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master's degree in Physical Therapy, or Master’s in a related field with a Bachelor's in Physical Therapy; or Bachelor’s in Physical Therapy with 5 years of PT experience. License evaluation for degrees earned outside the U.S. to be equivalent to a U.S. PT degree. Valid driver's license or reliable, independent transportation to patient residences. Posted salary range: USD $55.00 - USD $57.00 /Hr.
