Position Summary The Case Manager provides direct support to youth and families through coordination of services, treatment planning, and community linkage. This position works closely with therapists, residential staff, and external partners to ensure youth receive individualized, consistent, and supportive care throughout their stay.
Key Responsibilities & Role Highlights
Coordinate and implement individualized service plans in collaboration with the treatment team.
Serve as the primary contact for families, providers, and community partners.
Support youth in achieving treatment goals through skill‑building and strength‑based interventions.
Facilitate care coordination meetings and ensure timely communication among all stakeholders.
Link youth and families with community resources, aftercare, and support services.
Maintain thorough and timely documentation in the electronic health record system.
Participate in treatment team meetings and interdisciplinary collaboration.
Education & Experience Requirements
Bachelor’s degree in Social Work, Psychology, or related human service field required.
Prior experience working with children or adolescents in behavioral health, social services, or residential treatment preferred.
Strong communication, organizational, and problem‑solving skills.
Ability to work effectively as part of a trauma‑informed, collaborative team.
Valid driver’s license and access to reliable transportation required.
