Job Summary Provides physical therapy services to patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes under occasional guidance. Vanderbilt Health is seeking a compassionate and skilled Physical Therapist to join our Pediatric Home Health team on a PRN basis. How you'll make a difference

Deliver skilled physical therapy services to pediatric patients in their homes.

Develop and implement individualized care plans in collaboration with families and care teams.

Document patient progress and outcomes using EMR systems.

Participate in interdisciplinary case reviews and care coordination.

Qualifications

Licensed Physical Therapist in the state of Tennessee.

Experience in pediatrics, especially with medically fragile infants and children

Acute care or home health experience preferred.

Proficient in the use of Electronic Medical Records (EMR) systems for documentation and care coordination.

Strong communication and organizational skills.

Passion for working with children and families in a home-based setting.

CPR certification and valid driver's license required

Key Responsibilities

Analyzes comprehensive information from formal and informal assessment that is pertinent to the patient's care and management to determine a rehabilitation treatment diagnosis.

Collaborates with stakeholders in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the rehabilitation continuum.

Applies appropriate, evidence-based interventions to ensure effective outcomes and re-evaluates performance to assess patient's progress towards goals.

Employs strategies to promote successful return to home, community, and school/vocational environments.

Technical Capabilities

Rehab Therapist Patient Documentation (Novice): Documents in the appropriate templates and utilizes only approved abbreviations within patient documentation. Meets institutional standards for quality and timeliness of documentation.

Treatment Planning & Goal Setting (Novice): Develops and documents patient focused short and long term goals that are functional and measurable. Identifies the need for and referral to other services, as needed.

Rehabilitation Therapist Patient Evaluation (Novice):> Evaluates patients with physical deficits by obtaining pertinent assessment information, administering standardized tests, and documenting functional status to guide care.

Treatment Implementation (Novice): Provides treatment per physician prescription and plan of care, modifying as needed and supervising relevant staff; completes at least one year of clinical experience in treatment implementation.

Physical/Occupational Therapist Patient Education (Novice): Communicates functional impact of findings and treatments, and may supervise and train students or staff.
