Responsibilities and Responsibilities Summary

The Mental Health Screener provides care in the acute care setting within the scope of practice defined by the Screening Law and other regulatory standards. Duties include ensuring safety, coordinating with interdisciplinary team members, assisting the Patient Care Manager, supervising and directing task delegation to appropriate support personnel, and reporting safety issues as part of a High Reliability Organization to ensure patient, staff, and visitor safety. Qualifications

Required: MA/MS degree in psych or behavioral health related field, plus three years of prior experience in a psychiatric setting, one of which is in crisis setting. Must possess the ability to work in stressful situations. Must possess and maintain a valid N.J. Driver's License. Preferred: It is preferred that individuals hired be eligible to obtain their Temporary Screening Certification Number upon hire and must obtain their permanent number within one (1) year. 12:00 am - 8:00 am Including some Holidays and two weekend shifts per month Per Diem Essential Functions

Answers phones, including hotline and business line, in a professional and timely manner Communicates effectively with other members of the health care team (internal and external) Communicates with the Screening RN in Charge of Shift regarding the patient's treatment plan and disposition Performs thorough ongoing assessments according to departmental guidelines Provides patient care in a timely manner Demonstrates professional behavior according to hospital codes in the Conduct for Patients/Customers Describes screening process and updates progress of evaluation/interventions to patient/family Documents Presenting Illness with rationale for why the patient is here today Documents attempts to obtain insurance pre-certification Documents collaboration with patient and/or family in care planning and discharge Documents gathering of relevant collateral information Documents understanding of voluntary admission Documents that discharge planning information has been provided Documents thorough completion of off-site Screening evaluation requests Documents thorough completion of Community Outreach requests Documents outreach outcomes based on Screening Law Evaluates effects of interventions and updates the plan of care Prepares and maintains accurate records and reports in a timely manner (prior to end of shift) Provides hotline information/referral/interventions per Screening guidelines Determines most appropriate services/disposition per patient needs Includes clear description of dangerousness and mental illness per Screening Law and guidelines Benefits and Perks
