Overview
RN Clinical Manager - Assisted Living (also known as Wellness Manager) at Erickson Senior Living. Oversees the care performance of the teams within our 60-resident Assisted Living neighborhood and provides clinical care at the highest practicable level using the Erickson Person-Center Approach model of care. Location: Charlestown by Erickson Senior Living.

Compensation:
$95-110k per year, plus eligibility for annual bonus
How You Will Make An Impact
Ensure compliance with state, local, and company policies, procedures, and regulations.
Coordinate and provide clinical oversight and care to residents in Assisted Living and Memory Care, including medication management, pain management, wound care, nutrition, and respiratory care.
Partner with the Assisted Living Manager to complete resident pre-screening/admission, re-admission, and ongoing significant change of condition assessments. Reflect resident preferences, goals, and individualized clinical needs in assessments.
Maintain/update each resident’s individualized service plan in collaboration with ALM, care associates, therapists, and other team members.
Coordinate and monitor outside services (hospice, therapy, home health, etc.) and ensure plans of treatment are integrated into the resident’s service plan per regulations.
Track and analyze clinical quality areas (wound care, infection control, falls risk reduction, pain management, weight loss, dehydration) and implement corrective actions; evaluate results of actions.
Participate in continuing care meetings (Operations, Service Plan, Family, PI/RM/Safety).
What You Will Need
Current RN license in the state of operation
Minimum of 5 years of experience as a registered nurse
Management experience in Home Health, Assisted Living, and/or Memory Care settings preferred
Experience with electronic medical records (EMR) preferred
