Base pay range $18.00/hr - $18.00/hr
firsthand supports individuals living with SMI (serious mental illness). Our holistic approach includes a team of peer recovery specialists, benefits specialists, and clinicians. Our teams focus on meeting each individual where they are and walking with them side by side as a trusted guide and partner on their journey to better health.
firsthand's team members use their lived experience to build trust with these individuals and support them in reconnecting to the healthcare they need, while minimizing inappropriate healthcare utilization. We are changing the way society supports those most impacted by SMI through collaboration with our health plan partners.

The firsthand Guide, a blend of CPRS and Community Health Worker, applies a hands-on approach to address health-related social needs, improve self-management of chronic conditions, and navigate healthcare systems. They leverage their community knowledge and personal experience to inform interactions and collaborate closely with multidisciplinary teams to promote social stability, recovery, and health outcomes.
The role emphasizes support and services that promote whole health and autonomy, empowering individuals to participate in their care. Guides are empathetic, trained, and experienced in living with psychiatric/substance use disorders and navigating complex healthcare and community resources.

As firsthand Guide, you will:
Perform community outreach to individuals with serious mental illness, including home visits and visits to community locations such as shelters, hospitals, and drop-in centers.
Empower individuals to identify and pursue their recovery goals by modeling and teaching coping strategies.
Engage in care coordination to identify social needs and connect individuals with local resources.
Assist with gathering information for benefits and services, and facilitate hand-offs to team members for benefits navigation and self-management support.
Address immediate follow-up needs, such as post-discharge support.
Promote adherence to care plans and support medication and treatment adherence through motivational interviewing.

The required experience includes:
Lived experience with SMI and/or SUD.
Qualified as a Community Health Worker or willing to complete coursework within 3 months and pursue certification within 1 year.
Ability to operate a vehicle with an active driver’s license.
Proficiency in Google Workspace (Gmail, Sheets, Docs).
Strong communication and rapport-building skills.
Decision-making skills regarding work approach and priorities.
Ability to multitask in a fast-paced environment.
Respect for cultural and demographic diversity.
The preferred experience includes:
Experience in direct outreach with individuals with SMI.
Current Peer Recovery Specialist Certification.
Knowledge of a holistic health approach and trauma-informed care.
Knowledge of local community resources and systems, including behavioral health and criminal justice.
Interest in expanding knowledge of health conditions.
