Home Health Services – serving the following areas: Cleveland, Gaston, Rutherford Counties
Radius rule: Traveler’s permanent address must be 50 miles outside of home unit
Department Information
Typical Caseload: 6‑7 units/day; 30/week
Average Admissions completed/day: 2 admissions (weighted 2.40 units)
Qualifications
Minimum Years of Experience required: 1 year; Home Health experience is preferred
Will you accept a first‑time traveler? Yes – if they have had Home Health experience as a nurse
Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance: RN license, BLS, driver’s license, proof of vehicle insurance
Skills required: Homecare Homebase charting experience, Oasis, hospice philosophy, infection control practices, wound vacs, PICC line dressing changes, port access, IV infusions, drains, critical thinking skills, catheter insertion
Patient Types/Common Diagnoses: 18 y/o + (No Peds), wound care, wound vacs, Pleurx drains, cath (foley and SP), feeding tubes, IVs (peripheral and PICCs/Ports), Diabetes Mellitus, Heart Failure, Medication and Disease management
Support within the Department: Leadership, LPN/CNA, Peers, Case Management
Technology/Equipment: EMR – HCHB (Homecare Homebase)
Schedule Mon‑Fri 8:00 a.m. – 5:00 p.m.; weekends and holidays may be required and discussed during interview.
Travel Requirements
RTO: Manager approval
Ability to drive at night
Scrub Color: Navy
