Substance Use Disorder Case Manager

is seeking a dedicated and compassionate Substance Use Disorder (SUD) Case Manager to join our behavioral health team. This role focuses on supporting individuals with substance use disorders through commercial treatment pathways, ensuring continuity of care, and promoting recovery. The ideal candidate will hold a CDCA or QMHS credential and have experience navigating commercial insurance systems and treatment modalities.
Key Responsibilities
Conduct comprehensive assessments to determine client needs, strengths, and goals
Develop individualized treatment plans in collaboration with clients and clinical teams
Coordinate referrals to detox, inpatient, outpatient, and community-based services
Advocate for clients within commercial insurance systems to secure authorizations and coverage
Monitor progress and adjust care plans as needed to support recovery and stability
Facilitate psychoeducation and support groups as appropriate
Maintain accurate and timely documentation in accordance with agency and regulatory standards
Collaborate with multidisciplinary teams including therapists, physicians, and peer support specialists
Provide crisis intervention and support as needed
Ensure compliance with HIPAA, ethical standards, and agency policies
Qualifications
Active CDCA or QMHS certification in the state of Ohio
Minimum of 1-2 years of experience in substance use disorder treatment, preferably within commercial insurance settings
Knowledge of ASAM criteria, levels of care, and evidence-based practices
Strong understanding of case management principles and recovery-oriented care
Excellent communication, organizational, and problem-solving skills
Proficiency in electronic health records (EHR) and documentation standards
Ability to work independently and as part of a collaborative team
Preferred Qualifications
Bachelor’s degree in social work, psychology, or related field
Experience working with diverse populations and co-occurring disorders
Familiarity with motivational interviewing and trauma-informed care
