Description Harbor is currently looking to add a CPST/TBS Provider (Case Manager) to our mental health team in Bowling Green. Responsibilities Responsible for accurate and timely documentation including activity/progress notes, ISPs, DPS logs, incident report forms, and other necessary service documentation. Assess client needs and develop a strength-based ISP in collaboration with the client, family, and others. Provide crisis prevention and intervention. Offer support, education, and consultation to clients, families, and significant others. Monitor clients to identify and minimize psychiatric symptoms. Assist clients and families in increasing social support skills and networks. Coordinate evaluations and assessments by internal and external providers. Oversee and coordinate all services outlined in the ISP. Assist clients in accessing community resources. Requirements High School Diploma and 3 years of experience; Bachelor’s degree in a related field preferred. Proficient in computer use, including Microsoft Word. Complete required training on mental health, substance use, crisis response, and community systems within 90 days. For specialized programs, such as Developmental Pediatrics, relevant certifications like a 40-hour RBT course are required or to be obtained within 120 days. Valid driver’s license, reliable transportation, and insurable driving record. CPR/First Aid and NCI/CPI certifications required within 90 days.
