Transitional Resources for Children – Transitional Foster Care (TRC-TFC) — Case Manager in Bronx, NY. Reports to Lead Case Manager. Full-Time: 35 Hours per Week. Job Summary

The Case Manager assesses the needs of unaccompanied children in care, develops Individual Service Plans, screens for human trafficking concerns, facilitates the safe and timely release or discharge of children and youth, and documents the provision of services in case files according to ORR requirements, state licensing standards, and Abbott House’s policies/procedures. General Responsibilities

Assess each case and adapt service plan throughout the life of the case. Practice sound and productive counseling skills. Document all contacts appropriately in the case record. Write timely, clear, concise progress notes, assessments and other documents as required. Screens for human trafficking concerns and work closely with the PSA compliance worker. Identify and screen prospective sponsors, URM programs and Post 18 plans. Manage unification/discharge process. Concurrent planning to expeditate safe and timely releases. Coordinate inter- and intra-agency services to clients by working as a team with providers (foster parents, clinicians, teachers, medical, legal, FFS, CFS, CC, etc.). Create and provide each child a complete discharge package. After 30 days (and a 7-day call for a medical follow up) document the 30-day call. Close the complete UC file. Establish effective collegial relationships with Lead Case Manager, Senior Case Manager and Lead Clinician. Attend weekly staffing meetings and request meetings with the FFS for complex cases. Follow up on timely and appropriate referrals for medical services, health screening and mental health assessment of young people. Meet with the child face to face at a minimum of once per week (or more if needed) to provide case updates, phone calls and check-ins. Conduct home visits as needed and as required by city, state and federal requirements. Participate in the 7-day case management rotation. Make meaningful contributions to agency and department programming. Carry out the job responsibilities in a professional manner. Complete all required trainings. Mandated Reporter. Participate in individual, group, and department supervision. Attend and complete ORR & Abbott House Mandated trainings on a timely basis. Any other related duties as required. Physical Requirements

The employee is occasionally required to balance, stoop, kneel, crouch, or crawl. Computer – The Case Manager must be able to use a computer and learn software programs. Office Equipment – The Case Manager is required to operate a fax, copier, scan, and other office equipment. Communication – The Case Manager needs to be able to speak, read and write fluently in English. Writing – The Case Manager must be able to write in English with correct spelling, grammar, and punctuation. Lifting – The Case Manager may frequently lift 0-25 lbs., occasionally 25-50 lbs., and rarely over 50 lbs. Push/Pull – The Case Manager may frequently push/pull 0-25 lbs., occasionally 26-45 lbs., and rarely over 45 lbs. Sitting, Standing and Walking – The Case Manager will be required to sit, stand and walk throughout the work day. Stooping, Kneeling, and Crouching – May occur occasionally as defined. Work environment – Office setting with moderate noise; occasional increases due to program participant activity. Qualifications

BSW or BA/BS in human services required + experience in child welfare/child protective services. Excellent writing and communication skills including proficiency in both Spanish and English (written and oral). Demonstrated ability to reach out and work with community resources and foster parents. Hours and Time of Work
