Overview Founded in 1972, The Door empowers educationally and economically disadvantaged youth to reach their potential by providing a complete range of services all under one roof. Each year over eleven thousand young people from all five New York City boroughs come to The Door for primary health care, reproductive care and health education, mental health counseling, legal services, supportive housing, career & education services, daily meals, cultural arts and services for LGBTQ young people. The Door’s services are free, confidential, and geared towards youth aged 12-24.

Summary The Health Navigator is part of the health education team, they are responsible for aiding patients navigating medical and supportive service systems, including PrEP/PEP case management and in planning toward gender-confirming medical interventions. This position will work closely with medical providers, mental health staff, and health educators to ensure coordinated client-centered care.

Responsibilities

Provide case management and navigate care for clients receiving services in the health center, including PrEP/PEP, HIV treatment, gender-affirming care, and other supportive services, as needed.

Maintain electronic health records, case management logs, follow-up, referral tracking reports and other relevant documentation on PrEP/PEP, HIV treatment, pregnancy options, gender affirming care navigation and supportive services.

Provide individual and group health education and counseling with adolescents and young adults including psychosocial assessments, screenings, birth control, pregnancy prevention, HIV/AIDS prevention and making healthy choices for all AHC (Adolescent Health Center) clients.

Monitor data, schedule follow-up-up visits, escort patients to appointments, and provide other client centered support as needed.

Participates in health care delivery and case management, in collaboration with other health center staff, including the clinicians, social workers, lab staff, operations staff, nursing staff, medical assistants, and managers.

Provides HIV education, and pre- and post-test counseling and referrals to adolescents.

Keep current on changes that affect benefits and eligibility requirements, insurance and Medicaid enrollment and pre-authorization for prescriptions and specialists. Create linkages and referral resources with other Door programs and external service providers.

Assist in developing TGNC and HIV prevention educational and promotional materials, and social media posts.

Represent The Door during community-based assignments, events and meetings.

Aid in quality improvement through ongoing activities, assessments and work plans

Complete all mandatory staff training.

Perform other duties as assigned by supervisor

Qualifications

A bachelor’s degree in public health, social sciences, or a related field is required, however two years related experience can be substituted.

Experience working in LGBTQ services, HIV, PrEP/PEP, adolescent development, or health education

Ability to work both independently, and as part of an interdisciplinary team.

Ability to work well under pressure, exercise sound judgment, multitask, and meet deadlines.

Excellent verbal and written communications skills

Ability to remain calm, focused, and helpful under stress, excellent attention to detail required.

Demonstrated experience with at-risk youth, including LGBTQ youth of color.

An understanding and practice of the principles of positive youth development

Training in youth empowerment philosophy and harm reduction

Microsoft Office, EHR (preferably eClinicalWorks) and other software experience

Must be a professional and responsible person who is able to develop and keep interagency liaisons as a means of supporting client needs

Bilingual (English/Spanish or French) a plus!

Salary $50,000-$55,000

Hours 35 hours per week (some evenings); onsite position
