Under the direct supervision of the Director of Nursing, the Physical Therapist II, SNF is responsible for providing patient care services consisting of assessments, evaluation and implementation of modalities and treatments, and developing appropriate treatment plans. The PT will deliver care in the home setting in accordance with established departmental policies, procedures, and the licensing agencies in the State of California. The role also includes facilitating therapy scheduling and conducting Quality Assurance (QA) audits as requested by management staff.
Essential Duties And Responsibilities
Organize and conduct medically prescribed Physical Therapy programs.
Effectively utilize appropriate resources to ensure sensitive, personal care to individuals requiring physical therapy services.
Assess patients and provide appropriate treatment plans, performing individualized therapy programs that may include soft tissue massage/mobilization, traction, therapeutic exercise, manual techniques, durable medical equipment training, and safety assessments and recommendations.
Collaborate with other team members to ensure services and activities meet patient needs.
Apply diagnostic and prognostic muscle, joint, and functional ability tests.
Evaluate, document, and report patient progress for team review.
Ensure patients, caretakers, and families follow established policies and procedures at all times.
Interpret physical therapy policies and procedures to patients, caretakers, and family members as necessary.
Participate in peer feedback to improve performance of the line of service.
Practice fiscal responsibility and strive to maintain minimum targeted key performance indicators set by the Director of the department.
Extend courtesy and consideration to others in interpersonal relationships.
Demonstrate a professional manner in the workplace at all times.
Seek clarification of duties when needed.
Channel suggestions, criticisms, and questions to appropriate personnel.
Facilitate and report therapy audits related to the Quality Improvement process and QA program in cooperation with management staff.
Participate in staff meetings or review meeting minutes if not present.
Work with management to facilitate therapy scheduling, ensuring required therapy visits are performed.
Follow standard operational workflows established for the department (including preferred communication methods, EPIC documentation, and patient scheduling).
Establish good rapport with all team members, patients, and other health care providers.
Instruct, motivate, and assist patients to perform physical activities that may require assistive devices.
Participate in activities to meet the health needs of people locally, nationally, or globally as recommended by the American Physical Therapy Association (APTA) code of ethics.
Help facilitate and coordinate meetings, record minutes, and report for assigned specialty group.
Complete tasks for assigned projects to support existing service lines (e.g., scheduling, staff/ student education and training, QA, performance improvement, compliance, in‑services).
Assist in development of new service lines, strategic plan initiatives, and/or programs of the department.
Facilitate training and participate in mentoring, clinical education, and staff performance improvements.
Demonstrate system values in performance and behavior.
Comply with system policies and procedures.
Perform additional duties as assigned.
Qualifications Educational/Experience : Bachelor's Degree in Physical Therapy from an accredited program, licensed to practice, passed the National Physical Therapy Examination (FSBPT), and 2‑3 years of relevant experience.
Licenses/Certifications :
Basic Life Support (BLS) for Healthcare Providers – employee will complete RQI BLS curriculum within 3 months of hire.
California Physical Therapist (PT) – licensure upon hire.
Other Requirements :
Required: Bachelor’s or Certificate degree from an accredited Physical Therapy Program.
Preferred: Master’s or Doctorate Degree from an accredited Physical Therapy Program.
Bargaining Unit : EAP
Rate of Pay : $57.59/hour + DOE
