Physical Therapist – Hybrid Position (SNF)
We’re seeking a motivated and compassionate Physical Therapist to join our team in a hybrid role serving residents in a Skilled Nursing Facility (SNF) setting. This flexible opportunity is ideal for clinicians looking to combine the benefits of in-person patient care with the convenience of telehealth practice.
Position Details
Hybrid schedule: 2–3 days on-site; remaining days remote via telehealth Patient population: Skilled Nursing Facility residents, short-term rehab and long-term care Employment type: Full-time or part-time considered Start date: Flexible
What You'll Do
Evaluate and treat patients with a variety of physical impairments in alignment with individualized care plans Deliver high-quality therapy services both in-person and via telehealth platforms Collaborate with an interdisciplinary team to support patient outcomes and functional independence Document services in compliance with regulatory and payer requirements
Qualifications
Current Oregon Physical Therapist (PT) license (or eligibility) Prior experience in SNF or geriatrics preferred Comfortable with or willing to learn telehealth delivery Strong communication and clinical reasoning skills
