Base Pay Range $30.00/hr - $32.00/hr
Summary The LPN is responsible for providing direct nursing care to residents and supervising the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to ensure that the highest degree of quality care is maintained at all times. Full Time and Part Time available – Day and Night Shift.
Responsibilities
Direct the day-to-day functions of the nursing assistants; meet with assigned nursing staff and support personnel for shift planning and program execution; make written and oral reports/recommendations concerning shift activities; admit, transfer, and discharge residents as required; provide leadership to nursing personnel assigned to your unit/shift.
Complete and file required record‑keeping forms and charts upon resident admission, transfer, and/or discharge; receive and transcribe telephone orders from physicians and record them on the Physician’s Order Form; chart nurse’s notes descriptively and accurately; fill out accident/incident reports and submit to Director as required; perform routine charting duties in accordance with established policies and procedures.
Prepare and administer medications as ordered by the physician.
Develop and maintain a good working rapport with inter‑departmental personnel and other departments to ensure nursing services are adequately maintained and meet resident needs; create a warm, positive atmosphere within the unit and shift; receive and give the nursing report upon shift hand‑over; make rounds with physicians as necessary.
Administer professional services such as catheterization, tube feedings, suction, dressing/bandage changes, colostomy and drainage bag management, phlebotomy, range‑of‑motion exercises, care for dying or deceased residents, etc.
Monitor personnel to ensure compliance with safety regulations in the use of equipment and supplies.
Maintain confidentiality of resident care information including protected health information; report known or suspected incidents of unauthorized disclosure; ensure nursing care is provided in privacy and that personnel knock before entering a resident’s room.
Performs other duties as assigned.
Qualifications
Licensed Practical Nurse with current unencumbered state licensure.
Must be able to read, write, speak, and understand English.
Must be able to make independent decisions when circumstances warrant such action.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
Current CPR certification is required.
Experience in long‑term care is preferred.
