Case Manager, Social Worker – PRN

The Mercy Hospital in Ada, Oklahoma is a 159‑bed acute‑care facility that provides vital healthcare services to the Ada and surrounding communities.
How You’ll Contribute
Interview and assess patients and/or their family, caregivers, and/or legal representatives.
Determine, prioritize, and arrange internal and external services or interventions.
Participate in case reviews to evaluate case‑management progress and consult with the healthcare team to promote compliance with patient treatment plans.
Assist with discharge planning and identify resources such as transportation, housing, healthcare, and social/spiritual services, providing referrals as part of the discharge plan.
Mentor new staff, students, and interns.
Perform Medicaid screenings as required for the NHP program.
Assist patients with Safety Net applications as needed.

Qualifications
Bachelor’s degree or equivalent (required).
Licensed Social Worker – must obtain licensure within six months of hire.
Current state clinical licensure (required).
Basic Life Support (BLS) certification required within 30 days of hire.
Experience with case management, discharge planning, Medicaid screening preferred.
