Qualified Nutritionist - Chenango County
Program: Women, Infants, and Children (WIC) Reports to: WIC Director Status: Full-time, Non-Exempt Schedule: Monday 11am-7pm, Tuesday, Wednesday, Thursday & Friday 8am-4pm; 37.5 hours per week Starting Wage: $25 per hour Location: 26 Conkey Avenue, Suite 301, Norwich, NY 13815 - Chenango County Job Function
Provides nutrition education as well as counseling to low-income women, infants, and children with dietary, medical, and health risks by performing the following duties. Nutritionist Duties and Responsibilities
Conduct participant-centered nutritional assessments, including anthropometry, hematology, dietary, medical, and health information necessary for the WIC certification process. Provide and document participant-centered nutrition education/counseling (individual/family sessions and group discussions). Identify high-risk criteria and the need for an Individual Care Plan (ICP). Provide counseling for high-risk participants, ensure continuity of care, and determine when an ICP can be closed. Coordinate nutrition education, including breastfeeding promotion and support, tailored to each high-risk participant. Refer high-risk participants to other health-related and social services as necessary. Track progress of high-risk participants and document outcomes. Authorize and tailor food packages for participants. Review and approve formula requests and Medical Documentation Forms. Conduct breastfeeding assessments and provide counseling to support breastfeeding initiation and continuation. Refer clients to community services such as early childhood development, mental health, substance abuse programs, social services, and food pantries. Screen for lead and immunizations, providing referrals as needed. Ensure efficient assessment and education processes. Provide oversight and guidance to CPA and Nutrition Assistant staff. Lead in planning, conducting, and evaluating participant-centered nutrition services. Assist in training, orienting, and mentoring new staff. Develop, conduct, and evaluate nutrition training for LA staff. Assist with the development of the Annual Nutrition Services Plan. Assist with outreach activities and community events as needed. Work with the WIC Director to track data, review clinic flow, and manage other tasks. Attend trainings for professional development, including out-of-town travel if necessary. Complete annual Continuing Education hours as required. Provide education and support on the Farmers’ Market Nutrition Program and issue benefits to participants. Perform tasks such as answering phones, scheduling appointments, and managing eligibility questions. Collect data, create or locate records, and determine eligibility for WIC applicants. Review Rights and Responsibilities with participants and conduct program orientation. Qualifications and Requirements
Registered Dietitian Nutritionist (RD/RDN), OR a Bachelor’s, Master’s, or Doctoral degree in Nutrition from an accredited college or university; must receive approval of NYS Health Department to work as a CPA. Must have a valid NYS Driver’s License.
