Responsibilities:
Coordinate and link patients with additional services
Provide crisis intervention, stabilization, and brief counseling as needed
Provide proactive behavioral health case management to clinic patients
Position Details:
Employed position
Full-time, Monday through Friday work schedule
Fluency in an Asian language or Spanish is preferred but not required
Ability to work with limited-English-proficient populations
Master's degree in related fields
