The pharmacist dispenses prescribed drugs in a healthcare facility environment, transfers prescriptions, provides drug information in collaboration with other healthcare professionals, performs medication order entry into the computer, monitors patient profiles for appropriate medication use, oversees the work of the support staff, and documents relevant information. Performs clinical duties including medication therapy management and clinical monitoring with the goal of positive clinical patient outcomes. The position is expected to meet state certifications, requires familiarity with standards, concepts, practices, and procedures, and relies on experience and judgment to plan and accomplish goals.

Responsibilities

Reviews and interprets patient prescriptions and profiles to prevent overdoses, incompatibilities, and toxicity and to ensure high-quality pharmacy services and patient care; processes prescriptions accurately and efficiently.

Demonstrates understanding of formulary systems and ensures compliance with the formulary.

Documents all clinical interventions in the department-approved tracking system.

Supervises and reviews all functions performed by support personnel within the pharmacy.

Removes all out-of-date medication from inventory immediately upon discovery and checks affected patients to ensure safety.

Maintains physical inventory to ensure adequate supply of medications to meet patient demand.

Demonstrates understanding of pharmacy management and clinical programs.

Serves as the pharmacist in charge if required by the Louisiana State Board of Pharmacy.

Completes all assigned daily tasks and assists others when time permits.

Critical thinking: applies pharmacokinetics knowledge and pharmacological effects of drugs to patient care.

Computes calculations to originate clinical suggestions based on patient parameters (age, organ function, weight) for effective medication use.

Advises healthcare professionals on drug properties, administration, stability, and compatibility to ensure safe and effective care.

Utilizes references and computer programs to dose medications and understand pharmacokinetics.

Provides in-service education and participates in community education activities when directed.

Records and reports data on interventions that improve patient care, adverse drug reactions, medication errors, Drug Utilization Evaluations, and therapeutic interchanges.

Able to assess situations and choose appropriate courses of action.

Communicates effectively with diverse individuals, considering special needs.

Interpersonal relations: communicates with physicians, nursing staff, and customers about availability, side effects, and incompatibilities of drugs; engages with pharmacy practices through newsletters and management communications.

Manages support staff and provides direction in daily responsibilities.

Resolves communication gaps with healthcare professionals and coworkers promptly.

Participates in quality improvement initiatives and follows resulting processes.

Other duties as assigned or requested.

Qualifications

Education: Doctor of Pharmacy (Pharm.D).

License/Certifications: Licensed as a Registered Pharmacist in Louisiana (RPH).

Experience: Experience with 340B and Patient Assistance programs.
