Monday - Friday Job Summary Partners in Home Care is seeking a dedicated
Physical Therapist
to provide skilled evaluation and rehabilitation services to patients in their homes. This role includes direct care, care planning, patient/family education, and coordination with an interdisciplinary team to ensure high-quality, evidence-based outcomes. Key Responsibilities Evaluate patients and develop individualized care plans in the home setting Provide direct, skilled physical therapy per plan of care and physician orders Adjust treatment frequency and duration based on patient progress Complete and score OASIS assessments; collaborate with the Quality team Maintain knowledge of Medicare regulations, Conditions of Participation, and payer requirements Perform physical therapy interventions, including wound care when appropriate Document care and communication using point-of-care systems Coordinate with schedulers, optimize travel routes, and communicate availability Participate in care conferences and agency quality improvement initiatives Supervise Physical Therapist Assistants and CNAs as required Plan and manage admissions, re-evaluations, and discharges Maintain licensure and continuing education standards Uphold ethical standards and participate in agency training sessions Qualifications Bachelor’s degree or higher from an accredited Physical Therapy program Current license to practice in the State of Montana Valid driver’s license, auto insurance, and reliable transportation Access to high-speed internet 2+ years of experience preferred, especially in home health or clinical rehab Schedule Schedule may vary depending on FTE status and patient care needs, as directed by the supervisor.
