Position Summary “This position is assigned in direct support of the Medication Assisted Treatment (MAT) Mobile program and will contribute to its day-to-day operations, client engagement, and service delivery”.
Active working contributor to an integrated healthcare continuum that provides culturally competent, client centered, effective, coordinated, safe, affordable and accessible chronic and acute behavioral health related services in the community.
Essential Job Functions
Screen populations from various age groups with behavioral health needs
Completes and records initial and subsequent nursing assessment of each patient’s physical and mental status according to program protocols using interview, observation skills, and physical assessment techniques and communicates findings verbally and/or via written documentation
Assigns acuity levels based on risk stratification methods specified by the organization
Providestructure, and maintains a therapeutic environment in collaboration with the patient and other members of the healthcare team
Interveneduring psychiatric emergencies/patient crisis using evidence based practice strategies
Assistindeveloping positive relationships that support the client in meeting their personalgoals
Receivesand transcribes verbal and written provider’s orders according to established standards to ensure accurate treatment of patients
Offerscoaching on medication management
Administers medications and carries out physician's orders as prescribed
Evaluates therapies and care provided to individuals served
Participates in collaborative care team meetings
Actas a collaborative team member to any program within the organization, leading and addressing comprehensive health promotion and behavioral health maintenance needs
Provideinformation and/or referrals to resources in the community for increased support and stabilization
Actas aliaison between the individual served and external care providers as they transition within the continuum of care
Facilitatepreventative and restorative health and well-being education in individual, group and community settings
Reports to the Nursing supervisor according to the standards of care
Qualifications Certifications / Licenses
Current and unrestricted license as a Registered Nurse in the state of Nevada
Certified Addiction Registered Nurse or the ability to obtain licensure within a year
Ability to pass first level and NABS background clearance
During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you
Education
Bachelor’s degree required
Experience and Competencies
Previous experience working with individuals with serious and persistent mental illness, substance use/abuse, developmental disabilities, and/or other health conditions that impact behavioral health
At least one year of clinical experience; previous work experience in a related field may be substituted for clinical experience
Be able to utilize the organization’s electronic health record
Contributes to the development standards related to care and nursing procedures through participation on committees and assigned task groups
Participates in professional development activities and continuing educational programs to ensure currency in nursing practices and trends
Adheres to the nurse practice act of Nevada and all laws and rules governing the practice of registered nurses in order to maintain high standards of care and current licensure
Participates in performance and quality improvement measures to generate practice based evidence
