Salary: Starting at $50,000

Position Summary Unison Health is seeking a

Care Coordinator Intake Specialist

to engage clients and facilitate enrollment into the

OhioRISE care coordination program . This role involves initiating client engagement, verifying eligibility, scheduling appointments, and collaborating with community partners to ensure timely access to services. The ideal candidate has strong communication skills, crisis intervention experience, and a background in children’s behavioral health or related fields.

Key Responsibilities & Role Highlights

Conduct outreach and engagement with clients to enroll them in OhioRISE care coordination

Collect required documentation and proof of guardianship as needed

Collaborate with internal staff and community partners to facilitate urgent scheduling needs

Maintain accurate documentation and client records in compliance with agency policies and federal guidelines

Provide excellent customer service and support for children, youth, and families

Engage and enroll clients into the OhioRISE program

Maintain documentation of all contacts and services provided in the electronic record system

Ensure client confidentiality and proper handling of release of information in accordance with federal and agency guidelines

Education & Experience Requirements

High school diploma or GED required; Associate or Bachelor’s degree in Social Work, Psychology, or related field preferred

LSW licensure preferred

Minimum three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related human services/behavioral health field

Experience in at least one of the following areas: family systems, community resources, case management, child and family counseling or therapy, child protection, or child development

Ability to establish and maintain effective working relationships with clients, families, and community partners
