Review available resident information and evaluate its impact on the assessment and treatment process. Conduct and document a thorough functional resident assessment utilizing standardized procedures. Develop and implement an individualized treatment plan based on the resident’s needs and containing achievable functional goals. Comply with evaluation, treatment, and documentation guidelines and consistently maintain timely and accurate department records. Instruct Assistants in treatment procedures and provide close supervision while staff members are working with residents. Enter treatment and progress notes in the resident record following each treatment session. Revise treatment plans as needed throughout the treatment process. Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format. Orient residents to program services and treatment procedures appropriate to their needs. Work with nursing staff, residents, and families to maximize residents’ use of functional abilities. Instruct residents and family members or another caregiver about exercise programs and therapeutic procedures to be continued at home following treatment. Assist in the referral process when a resident requires additional services following discharge. Maintain current knowledge of community resources. Attend and participate in or provide information for rehabilitation team conferences regarding resident progress, problems, or needs. Attend and participate in other departmental and facility meetings as required. Access continuing education opportunities appropriate to discipline and responsibilities to maintain clinical competency. Participate in the quality improvement process. Act in compliance with Ahava Therapy Services’ regulatory and professional standards and guidelines. Adhere to facility policies and procedures and participate in facility quality improvement and safety programs. Perform other duties as assigned and required to provide quality care to residents. Understand reimbursement from various payers and allocate therapy resources efficiently based on resident goals and discharge destination-related outcomes. Knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines. Support the company’s mission and vision that projects a professional image. Independently perform duties and recognize situations where direct supervision is needed; willingly help others. Maintain the productivity level discussed and expected for your position. Qualifications
Bachelor’s or Master’s of Science degree in Physical Therapy. Current license in the state of practice. Must have BLS CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment. Job Details
