Full‑Time County Contractual – 40 hours per week

Budgeted Annual Amount:
$74,817.60
Benefits:
Participation in the County’s group health plan including prescription, dental, and optical, paid holidays and sick leave. Optional deferred compensation program at employee’s own expense.
Job Description
Conduct assessments using the interRAI, evaluating the health and psychosocial status of individuals referred to AERS.
Use nursing skills to recommend appropriate services, develop initial intervention plans, and recommend resources and programs.
Communicate and follow up with stakeholders, families, and clients as needed.
Manage schedule to accommodate competing priorities, cancellations, and client delays.
Develop comprehensive interdisciplinary plans of care, correlating findings and recommendations.
Follow up with other stakeholders (reportable events unit, support planners, nurse monitors).
Present cases in regular team meetings, share resources, and observe other assessments.
Complete ancillary documentation timely for LTSS Maryland, Apricot software, and paper files.
Participate in administrative requirements and special projects, completing at least one special project and seminar per year.
Education & Qualifications
Minimum Education:
Master’s degree in social work or related field from an accredited college or university.
Minimum Experience:
Two years in a non-hospital setting.
License/Certification:
LCSW or LCSWC.
Must be comfortable doing home visits and traveling throughout the county.
Desired Qualifications:
Experience in long‑term care, home health, and community health.
