VHCS Clinical - Therapy Job Summary:
Evaluate and treat patients with speech, language, voice, cognition, fluency, auditory (re)habilitation, and swallowing/oral feeding disorders. Educate and counsel patients and families. Provide independent therapeutic services and identify area(s) of clinical specialization and competency. Speech Language Pathologist (SLP) - Home Health PRN : Service Areas: Rutherford, Sumner, and East Davidson Counties. Schedule: Several days a week. Weekday availability preferred. Vanderbilt Health is seeking a dedicated and compassionate PRN SLP to join our Home Health team. How You'll Make a Difference: Provide skilled speech-language pathology services to patients in their homes. Evaluate and treat patients with speech, language, voice, and swallowing disorders. Develop individualized care plans and collaborate with interdisciplinary teams. Educate patients and caregivers on treatment plans and progress. Maintain accurate and timely documentation in accordance with Vanderbilt Health standards. Required Qualifications: Master's degree in Speech-Language Pathology from an accredited program. Current Tennessee SLP license (or eligibility). Minimum of 1 year of clinical experience in home health. CPR certification and valid driver's license required. Preferred Skills & Competencies: Clinical Expertise Strong knowledge of speech, language, cognitive-communication, and swallowing disorders. Proficiency in conducting assessments and developing individualized treatment plans. Familiarity with evidence-based therapy techniques and outcome tracking. Home Health Proficiency Experience working in home health or community-based settings. Ability to work independently and manage a flexible caseload. Comfort with travel and navigating various home environments. Communication & Interpersonal Skills Excellent verbal and written communication skills. Ability to build rapport with patients, families, and interdisciplinary teams. Cultural sensitivity and empathy in diverse home settings. Technology & Documentation Proficiency with electronic medical records (EMR) and mobile documentation tools. Strong organizational and time management skills. Ability to document accurately and efficiently in compliance with regulatory standards. Professionalism & Adaptability Self-motivated and able to work autonomously with minimal supervision. Adaptable to changing patient needs and scheduling demands.
