A Medical Care Manager contributes to AHF by utilizing their experience, skills, and education, along with a thorough understanding of the Ryan White Care Act Program within their specific area. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White client receives the support they require and access to available resources. This includes providing education and assistance to help clients understand their eligibility for the program. Working directly within the Healthcare Center, you will meet with clients, complete Needs Assessments, develop Action/Care Plans, and provide ongoing education about diagnoses and medications. For Ryan White contracts that include psychosocial needs, you will understand mental health needs and basic needs such as housing, food, and clothing, develop relationships with relevant programs and agencies, and facilitate referrals, support, and assistance for patients lacking these services. A Medical Care Manager enhances AHF's reputation by utilizing your case management experience to meet clients where they are, identify strengths and opportunities for growth, and assist with realistic goal setting. Your collaboration and engagement skills will help bring the essence of the Ryan White Care Act — the safety net for uninsured HIV/AIDS patients — to our most vulnerable populations. Your work will involve establishing Needs Assessments, Action/Care Plans, referrals, and support, directly contributing to AHF’s success in meeting the deliverables of our Ryan White contract and supporting measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services. You will be a passionate advocate for our top initiatives, ensuring that AHF’s Core Values and Mission stay at the forefront of your work. Our organization values agility and adaptability in a dynamic environment to provide the best experience for our clients and a positive workplace for our employees. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Medical Care Manager will work as part of a close-knit team, acting as a liaison between providers and clients, facilitating the transition from the exam room to implementing and executing the optimal care plan. You will provide brief interventions focused on achievable long-term goals, working together with clients towards viral suppression, better health outcomes, and improved quality of life. Licenses and Certifications: BS/BA in Social Work or other social service field with two (2) years of direct service delivery experience. Familiarity with housing case management. Knowledge of housing entitlement systems and procedures, including HOPWA Training Modules. AHF Commitment
