Job Responsibilities
Provides home visiting services within the 14-county health district to identify, assess, and deliver care coordination to eligible perinatal patients and qualifying infants.
Identifies high‑risk populations and tailors interventions based on needs and available resources.
Maintains HIPAA standards and provides outstanding customer service.
Under broad supervision, provides professional nursing care according to protocols, policies, and procedures; promotes the program through community engagement and outreach events.
Provides referrals and linkages to appropriate services when medical and environmental risks are identified.
Maintains CPR certification.
Responds to public health emergencies.
Minimum Qualifications
Associate degree or diploma in Nursing from an accredited college.
Current Georgia Registered Professional Nurse license.
Ability to lift up to 45 lbs. and bend/reach as required.
Official transcript prior to the start date.
Preferred Qualifications
Bachelor’s or Master’s degree in nursing, public health, or related field.
Previous employment with a state’s Department of Public Health or a County Board of Health.
Experience conducting home visits and patient home assessments.
Experience with intervention services for high‑risk newborns and infants.
Experience with case management and community outreach.
At least one year of labor and delivery experience.
Bilingual in Spanish and English.
Additional Information Must have a valid driver’s license; may require driving company vehicles and working evenings/weekends. Drug screening, criminal background check, and immunization records may be required. Male applicants aged 18–26 must provide proof of Selective Service registration. State Merit Application must be submitted by deadline.
