Facilitate communication between providers and patients, including assisting patients with technology to conduct virtual visits with team members Assess environment for safety concerns or hazards Communicate patient questions and needs (e.g., medications) to the patient care team Coordinate telehealth services to ensure seamless care delivery to patients at home, in clinics, or living communities (e.g., ALFs) Provide education and support to patients and caregivers on using telehealth technology effectively Maintain accurate records of telehealth appointments and patient data Collaborate with IT and technical support teams to resolve telehealth system issues Conduct training sessions to educate healthcare staff on telehealth best practices and workflows Identify technology needs and opportunities for improvement with cross-functional teams Troubleshoot and resolve technology issues promptly to minimize downtime Occasionally conduct in-person outreach to doctors’ offices, hospitals, and patients’ homes to educate potential patients and caregivers about Author Health’s mission and services Qualifications

Comfortable working with the senior population and those with behavioral health conditions Proven experience in a healthcare setting or similar role Knowledge of telehealth technology and software Excellent organizational and time management skills Strong attention to detail and ability to multitask Excellent communication and interpersonal skills Ability to work independently and collaborate effectively with team members Proactive and adaptable to change in a fast-paced environment Driver’s license and access to a car Preferred bilingual (Spanish-English) Willingness to travel up to 100 miles radius per day Preferred

At least 1 year of directly related experience Passion for and experience working with patients with mental health conditions Passion for and experience working with seniors Work Arrangements

This role requires walking to and from patients’ homes in all weather conditions. It requires a high level of flexibility, independence, and reliable transportation. In addition to in-person work, employees will occasionally work from home, using virtual tools and software to manage tasks, communicate with team members, and complete administrative duties. The ideal candidate will balance remote responsibilities with regular travel to ensure exceptional service and care in both settings. What We Offer
