Director, SNAP-Education at Adagio Health NUTRITIONIST

Adagio Health Inc is looking to hire one (1) full-time Nutritionist for its Armstrong County location. This position reports to the supervisor of the department and is an in-person position.

Adagio Health supports two key Nutrition Programs in Armstrong County: USDA's Special Supplemental Nutrition Nutrition Program for Women, Infants, and Children (WIC) and USDA's Supplementation Nutrition Assistance Program-Education (SNAP-Ed).

Women, Infants, and Children (WIC) Offering nutrition and breastfeeding counseling and education to pregnant and postpartum women, infants, and children up to age 5. You will provide nutrition education, assessments, and counseling of our WIC Participants, as well as being proficient in all aspects of certifying WIC Participants on the program. A typical day might include counseling and educating participants over-the-phone, offering breastfeeding support and referrals, tailoring food packages to ensure supplemental needs are met for each specific family, in addition to documenting and scheduling participants.

SNAP-Education Nutrition education centered on the 2020-2025 Dietary Guidelines for Americans and promotes a healthy lifestyle to a diverse population in a variety of settings. Our program reaches preschool, school-age, adult and senior members in our communities. You will also team up with community partners as well as school leaders to help improve the wellness of those participating in our program through Policy, Systems, and Environmental (PSE) change initiatives.

RESPONSIBILITIES Program Delivery

Initiates and delivers engaging nutrition education to a variety of audiences.

Initiates, leads, and provides technical assistance on PSE projects based on customized annual plan in conjunction with SNAP-Ed Coordinator.

Delivers indirect channels (food tastings, nutrition education reinforcement items, printed materials, etc.).

Determines program participant eligibility consistent with regulations and policies.

Completes nutrition assessment including the nutrition care plan, nutritional risk determination, and food package prescription.

Supports breastfeeding initiatives; Performs breastfeeding counseling and education in coordination with breastfeeding peer counselors.

Transports and assembles equipment required for satellite/off-site/partner-site program delivery.

Program Fidelity

Ensures all nutrition education is delivered as intended following Academy of Nutrition and Dietetics (AND) standards, program guidance, and USDA guidelines.

Provides individualized nutrition education to program participants using state agency approved counseling techniques.

Completes all required documentation, data tracking, and data entry by determined deadlines.

Provides high quality customer service, participant recruitment, and marketing interventions.

Makes referrals to outside social services as needed.

Builds relationships with and maintains regular communication with program partners.

Supports and participates in marketing activities being undertaken for the office/program.

Department Support

Works from/travels to Nutrition & Education Programs offices as needed.

Performs other duties as assigned.

MINIMUM QUALIFICATIONS Bachelor's degree in dietetics, human nutrition, food and nutrition, nutrition education, food systems management, public health, education or closely related fields from an accredited college or university. Strong interpersonal and coaching skills, and a desire to make a difference in the lives of our clients.
