Home Visits Physical Therapist — Full-time (Center Moriches, NY). Treat in home care with flexibility and opportunities for growth within the clinic’s leadership and development programs. Salary

$90,000 - $100,000 per year What We Offer

Plan, prepare, and carry out individually designed programs providing skilled therapy to patients in the home setting to restore function and independence Develop a plan of care/goals in consultation with the physician, family, and patient Timely and accurate documentation of visits Communicate discharge with family members and other team members Requirements

Home health experience is preferred but not required; mentor will be provided to assist with documentation and compliance Accept patients within your geographical area of coverage Adapt therapy plans to individual patient needs and setting Qualifications

Physical Therapist Program Graduate NY Physical Therapist License or eligible Ability to evaluate patient needs and develop an appropriate treatment plan Provide care in accordance with physician care plans, agency policy, and professional standards Instruct family members and other caregivers in therapeutic techniques CPR/First Aid certification required Metro invites interested candidates to join a passionate and dedicated professional community.
