Occupational Therapy Assistant (OTA) – In House (FT or PRN)
Join Evolve Therapy Services, LLC as an OTA to provide occupational therapy services in an in‑house skilled nursing facility in Rochester, MN.
Pay This position pays $30.00/hr – $40.00/hr.
Job Purpose To plan, organize, develop, and direct Occupational Therapy services in the facility in accordance with current federal, state, and local standards, guidelines, and regulations to ensure the highest degree of quality resident care.
Job Functions
Conduct regular screening of residents to determine the best intervention or treatment based on scope of practice.
Implement treatment plans, follow short‑ and long‑term goals, and achieve outcomes as established by the supervising therapist.
Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
Establish individual resident therapy schedules in cooperation with other team members.
Provide therapy per the established treatment plan and ensure proper documentation and billing of occupational therapy services.
Perform all other duties as assigned to effectively treat patients.
Comply with all CMS documentation standards of practice with point‑of‑care documentations as required.
Education & Qualifications
Associate’s degree in Occupational Therapy.
Active OTA licensure in state(s) of practice.
Ongoing continuing education and professional development to maintain licensure and certification.
