Care Facilitator, Bilingual Preferred-1191 (Maintenance) Fred Finch Youth & Family Services is a great place to work. Fred Finch Specialized Wraparound is a family-centered, community-based program that offers highly individualized mental health and behavioral services for youth and families referred by the Inland Regional Center (IRC) in Riverside County. These support services can include intensive care coordination with the family\'s extended team of providers, safety planning and crisis management, ongoing clinical assessments, direct behavioral and mental health treatment and intervention, and opportunities for psychotherapy and psychiatric and medication monitoring.
Responsibilities
Provide high quality, recovery oriented, strength based, culturally responsive, trauma-informed support services and care coordination to a diverse population of at-risk youth or young adults experiencing mental health symptoms, and their families.
Deliver individualized skill-building interventions to reduce symptoms or increase coping skills; provide transportation and linkage to community resources; and offer family support, including facilitation of Child and Family Team Meetings where applicable.
Perform services in developmentally appropriate, individualized formats across locations including homes, schools, program offices, psychiatric emergency settings, clinics, crisis residential programs, and shelters.
Document services, including patterns of behavior and incidents; participate in training and supervisory meetings.
Shift, Benefits Shifts Available: Full Time - Hourly; Mon-Fri 8am-5pm.

Qualifications Required
B.A. Degree in counseling, social work, or related field. Equivalent work and/or volunteer experience in the mental health field may be substituted for each year of required education.
At least one (1) year of experience of case management or helping youth or young adults to develop coping skills or manage mental health symptoms.
Certified to conduct a CANS/ANSA assessment within 90 days of employment if credentialed as a Mental Health Rehabilitation Specialist (MHRS) and CANS/ANSA is utilized by program.
Demonstrated ability to assess, triage and organize work; understanding of developmental milestones of children, youth, or young adults to provide care, treatment and services.
Knowledge of risk assessment, including danger to self or others, and effective community-based interventions for those at high risk.
Ability to utilize de-escalation techniques aligned with trauma-informed care in crisis situations.
Cultural responsiveness in working with diverse families and communities.
Clean driving record: current CA Driver\'s License or ability to obtain within 10 days after start; access to dependable transportation; personal car insurance meeting minimum liability coverage if driving for work.
Basic computer and keyboarding competence with the ability to use Microsoft Office Suite and/or electronic clinical documentation systems.
Preferred
At least one (1) year experience with program target population (examples: intellectually disabled, homeless, teens or young adults, seriously emotionally disturbed).
At least one (1) year experience with program modality (examples: school-based, residential, mobile/community-based, wraparound).
Applies strength-based, culturally responsive, trauma-informed approaches to supervision and participant care; familiarity with recovery-model orientation and public mental health/child welfare systems.
Exhibits strong behavioral, crisis management and facilitation skills.
Fluency in prevailing language(s) of the community.
