Description
Comunilife Inc.
is seeking an experienced Clinical Supervisor for its congregate program in the Bronx. The Clinical Supervisor will ensure all clinical services, including programming, case management, reporting, evaluations, case conferences, and audits, meet professional standards as outlined in Comunilife's Employee Handbook and regulatory requirements. The supervisor acts as Manager in charge in the absence of the Assistant Program Director (APD) and reports to the APD, working collaboratively with staff. Schedule:
Tuesday-Saturday, 9am - 5pm Essential Duties:
Assign cases to case managers, prioritize work, and schedule accordingly. Maintain records and supervision notes; prepare reports as required. Oversee quality assurance and compliance with policies and regulations. Audit client charts for service planning and progress documentation. Enforce and document home visit policies. Provide training to new and current case managers. Address client concerns and handle difficult situations. Participate in supervision with the APD and report on case status, staff performance, and trends. Conduct performance reviews for staff. Develop relationships with ancillary agencies. Prepare and submit program reports and statistics. Develop group programming in line with contracts and agency standards. Coordinate efforts to maintain occupancy rates and place clients promptly. Ensure assessments and service plans are developed and reviewed regularly. Oversee mental health and substance abuse services and referrals. Monitor and update client linkage agreements. Assist clients with obtaining benefits and services, including fiscal responsibilities. Supervise efforts to help clients locate housing and maintain independence. Complete assessments and conduct supervisory conferences. Coordinate intake interviews and report on referral statuses. Work with APD to ensure safe living environments and address building concerns. Attend staff and community meetings. Qualifications
Education and Experience:
NYS LMSW with at least 4 years supervisory experience in case management. Experience in counseling, especially with HIV/AIDS, mental health, or substance abuse populations, is required. Bilingual/bicultural skills in Spanish are preferred.
