Pre-Exposure Prophylaxis (PrEP) Case Manager
(Posted May, 2025) The Northeast Indiana Positive Resource Connection, an HIV/AIDS/Hepatitis Non-For-Profit service organization, is seeking a full-time PrEP Case Manager to join our dynamic team. Our PrEP Navigation Services provide high-quality, coordinated, client centered, and strength-based services consistent with our mission of preventing new cases of HIV/AIDS and Hepatitis and facilitating a compassionate and stigma-free community response in a 12- county region in northeast Indiana. Job Responsibilites
Responsibilities include but are not limited to: providing HIV, Hepatitis B and C, Syphilis, Gonorrhea and Chlamydia testing collaborating with area medical providers in prescribing PrEP to patients improving better health outcomes for HIV negative individuals through PrEP services assisting with access to health insurance and medication assistance programs enrolling qualified PrEP patients in the state’s secondary insurance program (PrEP MAP) scheduling medical appointments and sending out reminders of upcoming medical appointments and lab work retaining patients in PrEP navigation services until such time it is no longer needed Requirements
Bachelor’s Degree in human/social services or related field preferred Phlebotomy experience preferred but not required. Comfortable in performing blood draws Excellent written and oral communication skills Ability to work with diverse populations in a nonjudgmental capacity Positive outlook and professional demeanor Drivers License and ability to travel within our 12-county service area Preference for English/Spanish applicants but not required How to Apply
