Program Description Bridge Housing is intended to address the immediate shelter, and treatment needs of people who have serious behavioral health conditions and are experiencing homelessness. The site management and support services team are responsible in maintaining a 71-bed transitional recovery and community re-entry housing facility for those suffering from mental illness and/or substance use disorder who are experiencing homelessness. The target population often suffers from complex and co-occurring medical conditions and other characteristics that can challenge their ability to navigate mental health, physical health, and substance use treatment in the Santa Rosa community. This interim shelter is for individuals experiencing homelessness who have serious behavioral health conditions and/or substance use disorder issues who are willing to receive treatment. A particular focus will be Community Assistance, Recovery, and Empowerment (CARE) Court clients with a court-ordered care plan. CARE Court is aimed at helping those suffering from untreated mental health and substance use disorders that lead to homelessness, incarceration, or worse. Services include 24/7 medical and clinical staffing with a department assigned Housing Coordinator who will provide “gatekeeping” to assure appropriateness of all referrals. Referrals must be clients with a mental illness and/or co-occurring substance use disorder and are either precariously housed or are unsheltered. The proposed services will provide enhanced support to help homeless clients quickly reach permanent housing utilizing a client centered modality. A client-centered approach can also include a public defender and supporter to help make self-directed care decisions in addition to their full clinical team.

Specific Responsibilities

Provide care and clinical case management, mental health and substance use recovery and wellness services, and crisis prevention and intervention within the context of multidisciplinary team

Clinical Case Manager provides case management for clients who desire mental health/substance use recovery and wellness services, crisis prevention/intervention within context of a multi-disciplinary team

Utilize evidence-informed and evidence-based interventions to address guests’ needs and goals.

Collaborate with community partners acting as current access and intervention points

Meet documentation and other deadlines; comply with requirements for Medi-Cal, when applicable

Perform administrative tasks within timelines (chart completion, meeting attendance, etc.)

Connect guest with short and long-term support services such as housing assistance, public transportation vouchers, emergency food gift cards, and personal grooming and hygiene products

Meet documentation and other deadlines, complying with contractual deliverables

Assist guests in applying for _public benefits

Link guest to primary care services and other health care services

To reduce hospitalization, homelessness and incarceration, assist guests in re-integration back into the community

Develop and provide gender and culturally responsive curriculum

Develop and facilitate trainings targeting guests who have experienced domestic violence, sexual abuse, loss of identity, substance use, depression, financial hardship and housing displacement

Minimum Requirements and Qualifications

A minimum of 3 years of clinical case management, substance use disorder and mental health treatment experience for TAY and adults (including individuals experiencing co-occurring disorders)

Bachelor’s degree or higher in Social Work, Counseling, Psychology, or related clinical field

Associate level master’s degree & BBS Registered required

Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills

Knowledge of de-escalation, mitigation and pre-escalation techniques

A willingness to work from a strength-based, harm reduction, client-centered model with an emphasis on evidence-informed practices

Strong documentation skills, knowledge of Smart Care

Provide Support for Administrative staff in a multi-disciplinary setting

Other duties and responsibilities may be assigned as required to meet program goals and objectives

FLSA Status: Exempt

Salary Range: $80,000 - $100,000 Annually (DOE)
