You will play a key role in facilitating the transition from acute care to the community and supporting patients in re-engaging with their lifestyle activities.This is a part-time position (0.8 FTE / 30.4 hours per week) is on offer, with flexibility available.Key responsibilities:* Provide evidence-based physiotherapy care and service development* Maintain a broad understanding of healthcare factors for improved service and health outcomes* Supervise student and junior Physiotherapists* Support senior staff with referral intake and triage* Collaborate effectively within a multi-disciplinary team**About you:**Our Cabrini staff in the Rehab in the Home (RITH) team work collaboratively with their multi-disciplinary team and other healthcare professionals to provide high-quality physiotherapy services to patients. They are proactive, compassionate, and dedicated to improving patient outcomes.
